REQUIRED MEDICAL AND LIABILITY FORMS

MEDICAL INFORMATION RELEASE (download here)
Registration cannot be completed without a completed Medical Information Release form which can be downloaded here. Please fill out this form and either mail it to the camp office or bring it with you to registration. The Drury campus is located one block south of the Cox Medical Center. All emergency treatment will be handled through this hospital.

Participants of the Drury Jazz Camp must fill out the enclosed Liability Release form which can be downloaded here. This is a Drury University requirement for those attending any summer conference, camp, or workshop on the Drury campus. This form must be returned by mail or at registration.
